Purpose. This template is completed by the arranger during the arrangement meeting and, once signed, becomes the contract with the family and the instruction set for every other department.

1.How to use this template

Open a new record from the funeral case register entry so the case reference and deceased details carry across. Complete every section with the family present, read the summary back to them and obtain the signature of the authorised person. Save the signed record to the case and issue the itemised quote from it.

2.Authority to arrange

Record the full name and relationship of the person making the arrangements and confirm they are the executor or the senior next of kin. Where there is a dispute within the family, do not proceed until the Funeral Arranging Manager has reviewed the authority.

3.Registration details

Collect the details required to register the death, including the deceased's parents, marriages, children and occupation. Explain to the family that these details will appear on the death certificate and that corrections after lodgement take time.

4.Service details

Record burial or cremation, cemetery or crematorium, venue, date and time, officiant, music, readings, viewing arrangements, dress, coffin or casket selection, flowers, notices, memorial cards, catering and vehicles. Note any cultural or religious requirement and the person to contact about it.

5.Instructions for the mortuary and cemetery

The mortuary uses this record to plan preparation, dress and encoffining and to know whether a viewing is planned. The cemetery or crematorium uses it for the grave or cremation booking and the disposition of remains. Complete these sections precisely.

6.Costs and signature

Summarise the professional service fee, products and disbursements and the total, and record the deposit paid and payment terms. Both the family and the arranger sign and date the record.
