Grave shoring set B withdrawn from service
Hydraulic shoring set B failed its inspection on 2 September with a leaking ram and is tagged out at the Memorial Park depot. Do not use it. Set A is available and a hire set arrives on 15 September. No one enters a grave deeper than 1.5 metres without shoring in place. Report any concern to Terry Kowalski.

Identification checks: spelling matters
A coffin plate with a middle name spelt differently from the cremation permit was caught at the crematorium this week. The check worked, but the error started at arranging. From today, every arranger reads the full name back to the family letter by letter and every plate is checked against the arrangement record before encoffining.

Influenza and hepatitis B boosters
The occupational health nurse will be at Bayswater on 22 September and at the Memorial Park on 23 September for boosters and immunity checks. Transfer, mortuary and cemetery staff with a booster due have been emailed. Bring your record card. Anyone unsure of their status can ask Lin Zhao to check the training and vaccination file.

Cremator 2 service and rescheduling
Cremator 2 is due for its annual service and refractory inspection on 25 and 26 September. Only Cremator 1 will operate on those days, so cremation bookings will be spaced at 90 minutes. Arrangers, please check with Amara Osei before confirming a cremation time on those dates and tell families in advance where a committal time needs to move.

Quarterly internal audit: transfers and mortuary
The September internal audit covers the transfer and identification forms, the mortuary care register and the cool room temperature logs for August. Lin Zhao will sample twenty cases from 14 September. Please make sure every transfer form is filed in the case folder and every register entry has both identification signatures. Findings go to the leadership review on 29 September.
