Toolbox Talk Record Template

Purpose. This template records the safety talks held each week in the mortuary, cemetery, crematorium and transfer teams and provides evidence of consultation with workers.
1.How to use this template
The team leader completes one record for each toolbox talk. Choose one topic drawn from a recent incident, a seasonal hazard, a procedure change or a scheduled refresher. Keep the talk to fifteen minutes, invite questions and record every issue raised and the action agreed.
2.Attendance
Every worker present signs the attendance list. Workers who miss the talk are briefed individually within the week and their name is added with the date briefed. Students and contractors on site attend and sign.
3.Suggested topics
Topics rotate through the hazards in the WHS policy so each is covered at least twice a year. Team leaders will add topics that arise from the incident register.

- PPE and hand hygiene
- Sharps and exposure response
- Excavation shoring and confined space entry
- Cremator alarms and implanted devices
- Fatigue and first-call driving
- Heat and hydration for grounds work
- Emotional wellbeing and where to get support
4.Issues and actions
Record each issue raised in the worker's own words, the action agreed, the person responsible and the due date. Where an issue is a hazard, also complete an incident and hazard report form so it enters the register.
5.Filing
Send the completed record to the Compliance and Records Manager within two business days. Records are retained for five years and reviewed at the quarterly safety committee.
