Purpose. This form records the transfer of a deceased person into Everlasting's care, the two-person identification check, personal effects and the handover to the mortuary.

1.When to use this form

Complete one form for every transfer from a hospital, aged care home, private residence, the coroner or another funeral director. The form travels with the deceased and is filed with the case record. A separate internal movement record is completed for movements between Everlasting sites.

2.Identification check

Both crew members must check the deceased's identification against the releasing documentation before the deceased is moved, and both must sign. Where a wrist band or documentation is missing or does not match, the crew will stop and phone the on-call arranger. The tag number attached must be recorded on the form.

3.Personal effects

List every item of jewellery, clothing and personal property received with the deceased. Describe jewellery by colour and type, not by material, for example yellow metal ring with clear stone. Have the releasing person witness the list where possible.

4.Implanted devices and infection risk

Record any pacemaker, defibrillator or other implant and any infection risk advised by the institution or family. This information is required by the mortuary before preparation and by the crematorium before cremation.

5.Handover

The mortuary assistant receiving the deceased checks the tag and form, signs the handover section and enters the arrival in the mortuary care register. The crew files the form in the case folder before closing the transfer.
